Forms

In order to practice/tryout all students must have completed:

  • Activities Registration Form
  • Acknowledgement of the code of conduct (this is done when you complete the activities registration form)
  • Physical dated after May 1 2021 - there are 6 places a signature is required.  One on the front by a student and two on the front by a parent, one on the back by the doctor, one on the back by a parent, one on the back by the student.  Please use the WHSAA physical form below
  • $50 activity fee (JHHS)/ $25 (JHMS)
  • Travel release for entire season - Required for Alpine Skiing/Cross Country/Football/Golf/Nordic Skiing/Swimming
